The team consists of a Service Manager for Mental Health and Addictions, a Long Term Conditions Dietitian, and a Health Coach. Together, they provide essential support and guidance to individuals dealing with mental health challenges, chronic conditions, and overall wellness, ensuring equitable access to compassionate and progressive healthcare services. Their combined expertise supports healthcare professionals in delivering high-quality care, tailored to each individual's needs.
