With close to 100 centers, we are the leading provider of adult day health services with a  growing footprint and an expansive member population that stretches across the United States.

We have adult day health centers in Connecticut, Delaware, Illinois, Indiana, Kentucky, Maryland, Minnesota, Massachusetts, Mississippi, New Jersey, Ohio, Pennsylvania and South Carolina. Additionally, we have six home care branches in Kentucky and Massachusetts.

Active Day offers a day program designed to meet the unique needs of those who require health care and support services as a result of chronic conditions, such as: COPD; stroke; Alzheimer’s and other related dementias; seizures; diabetes; Parkinson’s; Multiple Sclerosis; hypertension and heart disease; developmental and intellectual disabilities. Through our specially trained staff, we are able to effectively manage these chronic conditions, allowing the member to prolong their independence and remain at their home in the community. Additionally, our comprehensive activities programs encourage socialization, provide cognitive and physical stimulation and promote overall wellness.
